Additional Client Questions
The FORM OPTIONS tab in PRF Settings gives shop owners and piercers complete control over what client info you collect before anyone steps up to the counter or gets into the chair. Built by working piercers and built on real shop feedback, these settings help you meet health department rules and ditch paper clutter while keeping client check-in fast and smooth.
Locked behind your Master Passcode, the FORM OPTIONS tab lets you set required client bio fields, greeting popups, automatic aftercare and waiver emailers, mandatory ID photos, custom client questions, and extra PDF attachments.
Accessing the FORM OPTIONS Tab
Since these settings control what health and legal data your shop gathers, you'll need owner access:
From the Welcome Screen, tap the Menu Icon (three horizontal lines in the top-right corner) or tap the Settings Gear Icon in the bottom-left corner of any main screen.
Enter your Master Passcode (default is 1234). No artist passcode will work here.
Tap the FORM OPTIONS tab along the top menu bar.
1. Welcome Message
Greet your clients before they start typing! Set up a custom pop-up that appears the moment a client starts check-in. Use it to post health department rules, age policies, studio guidelines, or just a clean welcome message.
Show Welcome Message: Flip this switch to gold to turn on your welcoming pop-up.
Add Welcome Message: Tap Add Welcome Message to open the text editor. Type your message directly or copy and paste text from another app on your iPad.
Tap DONE to save, or CLEAR to start fresh.
2. Automatic Emailer
Save paper, cut print costs, and keep your shop compliant by automatically emailing clients a digital copy of their signed waiver or your shop's piercing aftercare guide.
Tap Tap to configure under SET AUTOMATIC EMAILER to open setup:
Enable Automatic Emailer: Flip this switch to gold to turn on automatic emailing.
Email Waiver to Client: Turn this on to send clients a PDF copy of their signed release form as soon as they finish intake.
Note: Turning this on automatically requires clients to enter their email address during check-in so the app knows where to send it.
WAIVER EMAIL SUBJECT: Tap to type your custom subject line (like "Your Signed Waiver & Aftercare from ACME Piercing").
WAIVER EMAIL BODY: Tap to write the email message (thanking them, adding social links, or sharing aftercare notes).
> Important: Always hit the yellow SAVE button at the bottom to lock in your setup.
3. Form Requirements
This acts as your master checklist for the New Client Flow. Flip any switch to gold to make a field required or visible:
Show Optional Client Notes: Adds a box in the BIO section asking, "Client: What would you like to tell us?" Giving clients space to mention nerves, jewelry choices, or ear placement ideas helps your piercers connect with them before they sit down. Anything they type prints right onto the finished PDF waiver.
Require Client Email Address: Requires clients to enter a valid email address during check-in (this will activate automatically if Automatic Emailer is enabled so PRF knows where to send the email).
Show Health Questions: Asks clients about medical history, blood sugar, allergies, or conditions that affect piercing or healing. We strongly recommend keeping this on to protect your shop and cover your bases.
Request Gender: Adds an optional gender field to the bio screen.
Request Pronouns: Adds an optional pronoun field so your counter staff and piercers address clients comfortably and respectfully.
Require Age: Asks clients to type their age in addition to picking their birthdate on the date picker. Even though it feels redundant with their birthday on file, many local health inspectors require this specific field.
Require Photos of Both Sides of Gov’t ID: Crucial in states or cities where birthdates, barcodes, or renewal stickers sit on the back of driver's licenses. When enabled, PRF prompts the client to snap photos of both the front and back. Both images print directly onto the final PDF waiver in your cloud.
Capture Photo of ID: By default, PRF snaps a photo of the client's ID. If you pierce in a city, state, or country (like the UK) where snapping ID photos isn't customary or allowed by local code, turn this off. PRF will skip the camera and head straight to bio details.
4. Additional Form Settings
Need to hand out state-mandated piercing handouts, studio care guides, or extra consent pages? Skip the printer and upload custom PDFs directly into PRF to display during check-in.
Tap Tap to configure under ADDITIONAL FORM SETTINGS:
Enable the Additional Form Feature: Flip this switch to gold to turn on PDF attachments.
ATTACH ADDITIONAL FORM: Tap Additional form uploaded - Tap to change to pick your PDF from your iPad or connected cloud drive (like ACME-Piercing-Aftercare.pdf). A green SUCCESS! message confirms it's loaded.
Append the Additional Form PDF to the End of the Waiver: Turn this on to automatically attach your uploaded PDF to the back of the client's signed waiver file in your cloud—giving you instant proof during health inspections.
Require Clients to Read the Additional Form While Completing Their Waiver: Turn this on to make clients view and scroll through your document on screen before signing. This guarantees they had the info right in front of them if the law comes a-knockin '.
5. Additional Client Questions
If you want to ask your clients any random question, you pop it in here, and your clients will give you any of three sorts of answer formats. You can ask silly things, fun things, temporary things, and they will end up on the waiver. So watch that!
Tap Manage Questions under ADD QUESTIONS FOR CLIENTS to open the question builder.
Type your question into the box (like "How many active piercings do you currently have?" or "Have you eaten in the last two hours?").
Select Question Type: Choose Text (for written answers) or Number (for number inputs).
Tap + ADD QUESTION, then tap DONE.
Locking In Your Setup
When you finish setting up your FORM OPTIONS, scroll down the left sidebar and tap the gold SAVE & VERIFY button. The app saves your choices, backs them up to your cloud, and brings you right back to the Welcome Screen—ready for your next client.
