Piercing clients make their own waivers with PRF app without help from studio staff. The client steps up to the iPad, photographs her ID, and is led by the app through a smooth-flowing info-gathering interface.

Piercing Release Forms App: New Client Flow

Since new clients are excited about piercings, NOT paperwork, PRF app turns their waiver experience into an engaging five-minute lobby side quest. Without needing help from studio staff, the client steps up to the iPad, photographs her ID, and is led by the app through a smooth-flowing info-gathering interface.

The New Client Flow is designed based on feedback from thousands of clients and piercers across several countries since 2014. Most clients say it is easy and fun, and they “actually read a waiver for a change.”

Client-Driven Auto-Advancing UI

PRF programmatically moves the client to the next required input, triggered by prominent buttons and natural visual flow. This eliminates guesswork, and tapping the gold arrow at the top inspires intuitive backward and forward movement through the app.

The client will provide data in six distinct sections:

  • ID CAPTURE & ID VERIFY

  • BIO

  • THE PIERCING

  • SOCIAL

  • HEALTH

  • LEGAL

1. ID CAPTURE & VERIFY

Step 1: ID CAPTURE

The client is prompted to align their ID on the screen and tap the camera shutter button.

Camera Controls: The client can toggle the flash (lightning bolt icon) or flip between the front and rear iPad cameras.

Start Over: A button in the bottom left resets the app to the main welcome screen if needed.

Step 2: ID VERIFY

The app asks, “Can you clearly see all the details of your ID?”

The client selects their ID TYPE: State ID Card, Driving License, Gov ID Card, or Passport.

If the photo is blurry, they tap RETAKE. If it looks good, they tap USE PHOTO to move forward.

2. BIO

This screen gathers the basic biographical and emergency contact info required for a legally binding document. All fields are mandatory unless explicitly marked optional.

Basic Info: First Name, Last Name, GENDER (drop-down), YOUR PRONOUNS (drop-down), and DATE OF BIRTH (using a scrollable date picker).

Address: By default, CHECK FOR U.S. ADDRESS is checked, formatting the layout for US cities, states, and zip codes. Unchecking this adapts the fields for international postal formats.

Emergency Contact: The client must provide an EMERGENCY NAME and EMERGENCY PHONE NUMBER.

Once filled, the client taps PROCEED.


3. THE PIERCING

This section asks the client to describe what they are getting done today. Don't worry if they don't know the proper anatomical terms—they just need to give a casual description, and your piercer will lock in the exact details later on the back end. Read more about those settings here.

PIERCING DESCRIPTION: The client taps here to type a short, casual note (e.g., "Nose piercing").

PIERCING LOCATION: A drop-down menu categorized by body part (Facial, Ear, Navel, Surface, etc.) allows them to pinpoint the spot.

PIERCER: The client selects who is piercing them from your shop's staff drop-down.

Multiple Piercings: If they are getting more than one hole, they tap ADD ANOTHER PIERCING. (If they add one by mistake, they just tap the red trash can icon to delete it).

Tap PROCEED to continue.


4. SOCIAL

This screen tracks referrals and collects email addresses.

HOW DID YOU FIND US? A drop-down tracking tool (Instagram, Web Search, Referral, etc.). Depending on the selection, a secondary field may ask for their social handle.

YOUR EMAIL: If your studio settings require an email address for legal or health compliance, the app will explain that here and force an email input.

OPT IN: A switch that lets the client say, “Yes, please send me your newsletter and connect with me via social media!” If they leave it off, your shop may not use their email for marketing.

5. HEALTH

This section gathers vital medical history. The client must open each category to proceed.

ALLERGIES, DISEASES, & HEALTH CONDITIONS: Tapping these opens checkbox lists. The client must either check their specific conditions or check NONE and tap DONE.

ANY MEDICATIONS WE SHOULD KNOW ABOUT?: A drop-down where they select None or Yes, please list them (which opens a text box).

WHEN DID YOU LAST EAT?: A drop-down menu ranging from 0 hours to 8+ hours ago.

6. LEGAL

The final step is the liability release. To ensure clients actually read what they are signing, PRF presents your legal clauses one at a time.

The client taps the legal clause prompt, reads the pop-up text, and types their initials into the Your initials box. They tap DONE.

Once all clauses are initialed, they tap SIGN & COMPLETE.

A SIGN BELOW TO FINISH signature pad appears. They draw their signature with their finger and tap DONE.

The screen displays: NICELY DONE. PLEASE TELL STUDIO STAFF TO TAKE OVER FROM HERE. The app is now locked behind the Piercer Passcode, preventing the client from accessing any admin features.
